Application Requirements
To complete the Freshmen Online Admission Application, applicants must fulfill the following requirements:
- Grade 11 and Grade 12 Rating Cards: Provide Grade 11 Rating Card and Grade 12 1st Semester Rating Card, duly signed by the High School Principal. Include the Grade 12 2nd Semester Rating Card for comprehensive evaluation.
- Certificate of Good Moral Character: Obtain a Certificate of Good Moral Character from the previous educational institution, affirming the applicant's positive conduct and behavior.
- PSA/NSO Birth Certificate: Submit a copy of the official birth certificate issued by the Philippine Statistics Authority (PSA) or the National Statistics Office (NSO).
- ID Picture: Include one (1) piece of a 2 x 2 ID picture with a white background and a name tag for identification purposes.
- COMELEC Certification: Provide COMELEC certification of the parent (for applicants below 18 years old) and COMELEC certification of the student (for applicants 18 years old & above, issued after May 15, 2019).
- Learner Reference Number: Include the Learner Reference Number as part of the application details.
- Admission Requirement Form (ARF): Download, complete, and submit the Admission Requirements Form (ARF), ensuring that all necessary information is accurately filled out.
